Computed tomography and MRI in a 73-year-old woman with diplopia and dysarthria showed a solitary abscess of the brain stem, a rather uncommon cerebral lesion (1–4). Eleven days after onset of the symptomatology, her condition suddenly deteriorated. A rupture of the abscess was suspected and confirmed by repeat MRI.
