RESEARCH ARTICLE Reaction Patterns of TRAP and DBA.44 in Hairy Cell Leukemia, Hairy Cell Variant, and Nodal and Extranodal Marginal Zone B-cell Lymphomas Cherie H. Dunphy, MD airy cell leukemia (HCL) is a neoplasm of small B Context: The differential diagnosis of hairy cell leukemia (HCL) Hlymphoid cells with oval (or kidney-bean shaped) includes HC variant (HC-V) and marginal zone lymphoma nuclei associated with abundant cytoplasm with ‘‘hairy’’ (MZL). There is a high sensitivity of combined DBA.44/TRAP- projections in bone marrow (BM) and peripheral blood, positivity (+) in confirming HCL. A previous study of mucosa- diffusely infiltrating BM and splenic red pulp, and associated lymphoid tissue lymphoma showed DBA.44+ in strongly expressing CD103, CD22, and CD11c. The 10%, TRAP+ in 29%, and DBA.44+/TRAP+ in 5%. exact subset of B cells from which hairy cells are derived, is still unclear. However, previous studies have suggested Objective: We now study nodal MZL (NMZL) and HC-V. origination of hairy cells from the B lymphocytes Design: Two HCL, 2 HC-V, 3 MZL of bone marrow (BM), normally residing in the marginal zone of the splenic 2 MZL versus B-cell lymphoma, not otherwise specified (BCL, 2,3 white pulp. NOS) of BM, and 4 NMZL
