This review describes a lipodystrophy syndrome often seen in recent years in the treatment of HIV-1-positive patients. The syndrome consists of a redistribution in fat, hyperlipidaemia, and insulin resistance, and potentially undermines an important part of the enormous success that the introduction of potent antiretroviral therapy has gained. We describe the different aspects of the syndrome, its implications, and its potential aetiology.
