In Deutschland hat jeder zweite Linksherzkatheter keine unmittelbare interventionelle oder operative Konsequenz. Ein wesentlicher Grund für die eingeschränkte Indikationsqualität vieler Herzkatheteruntersuchungen liegt in der Unschärfe der nichtinvasiven Vordiagnostik, die sich in Deutschland überwiegend auf die klinische Symptomatik und das Belastungs-EKG stützt. Das Belastungs-EKG hat aber erhebliche Limitationen; Hauptprobleme sind die fehlende Ergometrierbarkeit zahlreicher, insbesondere älterer Patienten und die fehlende Interpretierbarkeit des Belastungs-EKGs bei bereits pathologischem Ruhe-EKG. Nach der Nationalen Versorgungsleitlinie Chronische KHK (NVL KHK) aus dem Jahr 2006, die erstmalig für Deutschland die evidenzbasierten diagnostischen Algorithmen aus den Leitlinien des American College of Cardiology und der American Heart Association (ACC/AHA) übernommen hat, kommen als gleichberechtigte bildgebende Verfahren die Stressechokardiographie mit körperlicher oder pharmakologischer Belastung, die SPECT-Myokardszintigraphie mit körperlicher oder pharmakologischer Belastung, die Dobutamin-Stress-Magnetresonanztomographie (DSMR) oder die Myokard-Perfusions-MRT mit pharmakologischer Belastung in Frage. Grundsätzlich ist kein bildgebendes Verfahren den anderen Methoden diagnostisch eindeutig überlegen, jedoch hat jedes Verfahren spezifische Vor- und Nachteile, die bei der Indikationsstellung an den individuellen Patienten angepasst werden sollten. Entscheidend ist der gesicherte hohe negativ-prädiktive Wert eines normalen Stress-Imaging-Befundes von 99 %, gleichbedeutend mit einer sehr niedrigen kumulativen Wahrscheinlichkeit (< 1 %) für einen kardialen Tod oder einen Myokardinfarkt innerhalb von wenigstens 12 Monaten, so dass i. d. R. innerhalb der nachfolgenden 12 Monate auf eine invasive Koronarangiographie verzichtet werden kann. Im Gegensatz zu diesen etablierten und evidenzbasierten Empfehlungen der NVL KHK, bei denen die funktionelle Ischämiediagnostik im Mittelpunkt steht, haben sich in den letzten Jahren in vielen diagnostischen Zentren eigenständige, nicht evidenzbasierte Algorithmen entwickelt, in denen die morphologische Koronardiagnostik mittels CT-Koronarangiographie die Funktion der funktionellen Ischämiediagnostik übernommen hat. Über den gesicherten prognostischen Stellenwert des Kalzium-Scores hinaus fehlt aber bislang die wissenschaftliche Evidenz für eine diagnostische Überlegenheit der morphologieorientierten CT-Koronarangiographie im Vergleich zur Ischämiediagnostik mittels bildgebender Verfahren. Ein innovativer Ansatz einer modernen Stufendiagnostik stellt der vom britischen National Institute for Health and Clinical Excellence (NICE) im Jahr 2010 dem National Health Service (NHS) empfohlene Algorithmus für Patienten mit mittlerer KHK-Vortestwahrschenlichkeit (10–90 %) dar. Patienten mit einer KHK-Vortestwahrscheinlichkeit von 10–29 % sollen demnach als First-line-Methode zunächst ein CT-basiertes Kalzium-Scoring, Patienten mit einer KHK-Vortestwahrscheinlichkeit von 30–60 % zunächst ein funktionelles Stress-Imaging-Verfahren und Patienten mit einer KHK-Vortestwahrscheinlichkeit von 61–90 % wie bei einer KHK-Vortestwahrscheinlichkeit über 90 % möglichst direkt eine invasive Koronarangiographie bekommen.
