Anterior Spinal Cord Ischemia after Infrainguinal Bypass Surgery Tomislav Mihaljevic, MD, and Michael Belkin, MD, Boston, Massachusetts Morbidity of infrainguinal bypass surgery is mostly related to cardiac complication, graft throm- bosis, and wound infections. Neurologic complications in these patients are unusual and gen- erally related to traumatic peripheral nerve injury or ischemia. Anterior spinal cord ischemia, manifest clinically as flaccid paraplegia or lower extremity paraparesis, is a complication usually associated with aortic surgery. Reported here is a case of an 81-year-old male who developed spinal cord ischemia after infrainguinal bypass under epidural anesthesia. blood loss was minimal. The patient experienced one CASE REPORT brief episode of hypotension with a decrease in systolic An 81-year-old male with a long-standing history of se- pressure to 90 mmHg, which was corrected by the bolus vere peripheral vascular disease and ischemic left foot rest of inravenous ephedrine. The patient was transferred to pain was admitted to the Division of Vascular Surgery in the recovery room in stable condition. During the initial November 1999.
