An Enabling Procedure: Laparoscopic Cholecystectomy 620 Correspondence Annals of Vascular Surgery ideal ‘‘enabling procedure’’: laparoscopic chole- cystectomy. This very common operation was not overly difficult to learn and yielded a dramatically better recovery for all patients. The high patient appeal increased the demand for the operation, and provided the initiate laparoscopic general surgeon with consistent and frequent practice in laparos- copy. It is also notable that laparoscopic suturing is not required for cholecystectomy and other basic laparoscopic procedures. Avoiding this technically challenging task helped make basic laparoscopic surgery accessible to all surgeons. The biggest challenge to the widespread per- formance of vascular laparoscopic surgery may therefore be the ability of the average vascular surgeon to perform enough routine laparoscopic procedures to gain and maintain the technical expertise needed to perform more complex oper- ations such as aortic bypass. The exposure and suturing skills needed for this type of procedure are very demanding to learn and require consis- tent practice to maintain. Without an enabling laparoscopic vascular procedure similar to laparo- scopic cholecystectomy, advanced laparoscopic vascular surgery might remain confined to spe- cialized centers.
