WIETERSEN A highly accurate differentiation of the benignancy or malignancy of small colonie polypoid tumors beyond the reach of the endoscope is possible by roentgenographically assessing th e growth behaviour of such tumor s on progress or serial barium enema studies. Obviously, if such an evaluation is possible with practically no risk to the patient, one will not be able to justify surgical removal of all polyps beoynd the reach of the endoscope. The advisability of surgical treatment of patient s harborin g one or more small polypoid tumors can be predicated on the basis of such an assessment. This becomes especially im portan t in elderly patients in whom there is a much higher incidence of polyps an d in patients with medical complications that increase the operative risk. Presently a strong trend has developed in the American and European litera ture advocating conservative management of smaller colonie polyps based on roentgenologic assessment of growth (FIGIEL et coll. 1962, FINKELSTEIN et coll. 1963, SPRATT et coll.
