Integrated care pathways - the touchstone of an integrated service delivery model for Ireland While health-care reform across many developed countries includes the objective of achieving integrated health care as a key goal, there is no one delivery model available on which all systems can draw upon to support the achievement of integrated health care. The evidence to date highlights thateven in the most advanced health-care systems, there is a substantial gap between what is known to work and what is provided. This paper discusses the potential for integrated care pathways (ICPs) to contribute to the reform agenda in Ireland to achieve effective and sustainable integrated service delivery. Abstract
