Background: Publications in high impacting, peer-reviewed journals are the main determinants of research rating and funding. The present study examines the geographic origin of publications in the field of urology. Methods: The 5 highest impacting urological journals were selected over 5 year period. Articles with abstracts were totalled according to country of corresponding author. Publications (total and corrected for population size) were assessed according to country and the percentage of gross domestic product (GDP) spent on research. Correlation was determined using Spearman rank. Results: A total of 11,109 articles were identified. Twenty countries produced 92.3% of these: USA (41.9%), UK (8.9%) and Germany (6.7%). When corrected for population size smaller European countries topped the ranking: Austria, Sweden and Netherlands. When corrected for percentage GDP spent on research: Czech Republic, Singapore, and New Zealand. The percentage GDP of a nation spent on research positively correlated with number of publications (r = 0.41, p = 0.014). Conclusion: The USA is the most productive country in terms of absolute number of urological publications. When population size is taken into consideration, smaller European countries were more productive. Increased investment in scientific research is associated with higher impacting publications in the field of urology.
