The study was conducted within the project realised at the University of Physical Education in Warsaw titled "The improvement of didactic and scientific potential in the field of occupational therapy as a key to the development of higher education" co-financed by the European Union within the European Social Fund. Abstract Introduction: Multiple sclerosis is a disease attacking the central nervous system. It develops when the myelin sheaths of nerve fibres, glial cells and immune cells are damaged [3]. The aim of this study was to present the project of adapting a bathroom to the needs and expectations of a person with MS and to show its implementation. Method: A direct interview with an MS client was conducted with COPM questionnaire (often used in occupational therapy) and medical documentation was analysed. Case study: Tadeusz (the client) was a 46-year-old man. He was diagnosed with MS when he was 43 years old. However, the first symptoms occurred two years earlier. In 2007 he underwent magnetic resonance and spine puncture examinations. The results confirmed a primary progressive type of MS. Identification of the client's problem: the need of independence while washing, shaving and bathing. Tadeusz was not able to take a shower by himself because of the fear of losing balance and falling down in the bathroom. The weakness of the left hand muscles and the lack of support rails made it impossible to use the bathroom independently. During several meetings with the client, a plan of adapting his bathroom (fixing handrails and a wooden step) was developed and the manner of using these devices was practised. Conclusions: Two weeks after the bathroom adaptation was finished, it was assessed together with the client. It was concluded that the adaptation meets his expectations and the aim is being gradually achieved. In the reassessment process, an improvement by 3 points was noted. The "task performance" and "satisfaction" criteria were given 7 points (in the scale from 1 to 10) by the client. However, in the first assessment he evaluated these criteria by giving them 4 points each. Nie uwaa si jednak, e zachorowanie na stwardnienie rozsiane skraca ycie, aczkolwiek choroba ta znaczco obnia jego komfort i jako [2]. Stwardnienie rozsiane jest chorob, która dotyczy komórek nerwowych (neuronów), komórek glejowych (oligodendrocyty) i odpornociowych mózgu, w których na skutek toczcego si procesu chorobowego zostaje uszkodzona oslona mielinowa. W rezultacie powoduje to nieprawidlowy przeplyw impulsu nerwowego pomidzy komórkami nerwowymi mózgu i rdzenia krgowego [3]. SM jest przewlekl, nieuleczaln chorob centralnego ukladu nerwowego, w wyniku której niszczona jest tkanka nerwowa. Po raz pierwszy chorob opisal francuski neurolog Jeana Martina Charcot, w 1868 roku. Opisal równie zmiany Multiple sclerosis is a rare disease, i.e. a disease that occurs in 5 per 10,000 cases [1]. It is an autoimmune and incurable disease. However, it does not shorten life but significantly reduces its comfort and quality [2]. Multiple sclerosis is a disease affecting neurons (neuronal cells), glial and immune cells whose myelin sheaths have been damaged. It results in improper passing of neuron impulse throughout brain cells and spinal cord [3]. It is a chronic, incurable disease of the central nervous system in the course of which nervous tissue gets damaged. It was first described by a French neurologist Jean Martin Charcot in 1868. Its name points to the fact that pathological processes connected with the disease appear in different parts of the nervous system. The symptoms are also scattered in time. The researcher indicated three main symptoms, i.e. dysarthria (speech disorder manifesting itself in slow and unclear articulation), ataxia (physical coordination disorder) and muscular tremor. He also described cognitive disturbances in MS patients, i.e. a significant "loss of memory, slowed down thinking and learning difficulties" [4]. Stwardnienie rozsiane moe wywolywa jeszcze inne objawy, do których zalicza si: zaburzenia ruchowe, czuciowe (drtwienie, cierpnicie lub mrowienie rk i nóg), zaburzenia widzenia (niewyrane lub podwójne widzenie), zespoly bólowe, zaburzenia poznawcze i nastroju, przewlekle zmczenie (poczucie niemocy, uniemoliwiajce aktywno intelektualn i fizyczn), slabo mini i ich wzmoone napicie, problemy z ukladem pokarmowym i moczowym [4]. Styl ycia bynajmniej nie ma wplywu na rozwój tej choroby. Nie ma te jednoznacznych dowodów na to, e jest to choroba dziedziczna, aczkolwiek ostatnie wyniki bada dowodz, e chorob wywoluje kombinacja czynników genetycznych i rodowiskowych. Z cal pewnoci mona jednak stwierdzi, e stwardnienie rozsiane nie jest chorob zakan [5]. W literaturze wskazuje si na cztery typy tej choroby: posta remitujco- nawracajc, wtórnie postpujc, pierwotnie postpujc i posta postpujconawracajc [5]. Statystyki dotyczce stwardnienia rozsianego s nastpujce [2;5]: 1. na calym wiecie choruje na SM okolo 2,5 mln osób; 2. liczba chorych w Europie wynosi okolo 500 tysicy; 3. w Polsce yje okolo 40 tysicy osób z SM (jeden chory na 1000 zdrowych osób); 4. okolo 75% cierpicych na SM w Polsce przechodzi na rent w okresie niespelna dwóch lat od momentu zdiagnozowania choroby; 5. SM jest chorob, która dotyka przede wszystkim ludzi mlodych, midzy 20. a 40. rokiem ycia, ale zdarza si te u osób poniej 18. roku ycia, a nawet u dzieci; 6. prawdopodobiestwo wystpienia SM u osób mieszkajcych dalej od równika jest wiksze (dowodem na to jest ogromna liczba zachorowa w krajach skandynawskich). Aktualnie, nie ma jeszcze leków, które moglyby skutecznie leczy SM, ale s takie, które hamuj rozwój tej choroby. S to leki immunomodulujce (np. interferon), które modyfikuj prac systemu immunologicznego. Terapia lekami znaczco obnia liczb ataków i zaostrze w przebiegu choroby. Interferon jest lekiem bardzo skutecznym, ale równie kosztownym, i dlatego w Polsce nie wszyscy mog mie dostp do takiego leczenia. Aby otrzyma refundacj kuracji interferonem, polski pacjent musi zosta zakwalifikowany do leczenia przez Narodowy Fundusz Zdrowia (NFZ), i spelnia okrelone kryteria: wiek poniej 35. r. .; czas trwania choroby; dotychczasowa (numbness or tingling sensation in arms and legs), visual disorders (unclear and double vision), pain, cognition and mood disturbances, chronic fatigue (general weakness that render physical and intellectual activity impossible), muscle fatigue and spasticity or problems with digestive and urinary systems [4]. The development of this disease is by no means influenced by lifestyle. Nor is there any explicit evidence for its genetic causes. However, the latest research has proved that it is caused by a combination of genetic and environmental factors. It can certainly be concluded that it is not a contagious disease [5]. In the literature of the subject there occur four types of this disease, i.e. relapsing-remitting MS, secondary progressive MS, primary progressive MS and progressive-relapsing MS [5]. Statistics concerning MS are as follows [2,5]: 1. there are approximately 2.5 million people with MS in the world; 2. there are approximately 500,000 people with MS in Europe; 3. there are approximately 40,000 people with MS in Poland (one case per one thousand healthy individuals); 4. ca. 75% of those who have MS in Poland start receiving health benefits within two years from the diagnosis; 5. MS is most often diagnosed between the age of 20 and 40 but it may also occur in teenagers below 18 or even among children; 6. the risk of developing MS rises with the increasing distance from the equator (illustrated by an immense number of MS cases in Scandinavian countries). Currently, there is no cure for MS but there are drugs which can slow down the development of the disease. They are called immunomodulatory drugs (e.g. interferon) as they modify the work of the immune system. Drug treatment significantly reduces the number of attacks and exacerbations of the disease. Interferon is a very effective yet costly drug and, therefore, not all individuals with MS can gain access to such treatment in Poland. In order to receive free interferon treatment, a Polish patient has to be qualified by the Polish National Health Service (NHS) and meet certain criteria, such as age below 35, duration of the disease, the number of previous attacks or particular types of symptoms. In Poland, only ca. 8% of MS patients are treated with interferon funded by NHS, whereas in other European Union countries the number reaches approximately 30%. Thus, Poland is in the last but one position as far as accessibility to interferon therapy is concerned [5]. liczba rzutów choroby; rodzaj objawów. Stwardnieniu rozsianemu towarzyszy wiele objawów, które znaczco obniaj jako ycia chorego. Dlatego dobrze i systematycznie prowadzona rehabilitacja, cho nie ogranicza istotnie kolejnych rzutów choroby, ani te jej nie hamuje, to jednak poprawia zarówno obiektywne wskaniki zdrowia, jak i subiektywnie oceniane samopoczucie. Terapia zajciowa jako jeden z rodzajów rehabilitacji chorego na SM okazuje si mie ogromne znaczenie dla poprawy jego niezalenoci podczas wykonywania codziennych czynnoci, a take usprawniania funkcji poznawczych [6]. I, chocia nie okrela si jednoznacznych wskaza czy zalece co do najbardziej skutecznej formy terapii w przypadku osób cierpicych na stwardnienie rozsiane, to badania jednak potwierdzaj istotne rónice dotyczce stanu zdrowia u osób poddawanych terapii i nierehabilitowanych [6]. Celem niniejszego opracowania jest przedstawienie i opisanie autorskiego projektu na przystosowanie lazienki do oczekiwa i potrzeb osoby chorujcej na SM, zaprezentowanie jego wdroenia oraz omówienie procesu terapii zajciowej przeprowadzonego z klientem chorujcym na stwardnienie rozsiane. Opis przypadku MS is accompanied by numerous symptoms which significantly decrease the patients' quality of life. Thus, although a proper and regular rehabilitation does not limit the number of further attacks or inhibit the development of the disease, it improves both objective health indices and subjective well-being. Occupational therapy is one of the types of MS patient rehabilitation which seems to be really significant in the process of improving their independence during everyday activities and improving cognitive functions [6]. Despite the fact that there are no unanimous indications or recommendations regarding the most effective form of therapy for MS patients, the research confirmed significant differences in the state of health between patients who undergo rehabilitation and those who do not [6]. The aim of this study was to present and describe the project of adapting a bathroom to the needs and expectations of an MS client, to show its implementation process and to discuss the process of occupational therapy conducted with the MS client. Pan Tadeusz jest onaty i ma dwóch doroslych synów. Mieszka z rodzin w domu jednorodzinnym, w miejscowoci Mociska poloonej w pobliu Warszawy. Cierpi na stwardnienie rozsiane, które zdiagnozowano u nie- The work presents a case study of a man with MS prepared on the basis of the Canadian Practice Process Framework (CPPF), which is widely used in the process of occupational therapy [7]. CPPF includes certain steps which an occupational therapist takes with a client during consecutive therapeutic sessions. The client gave his consent to participate in an interview and occupational therapy, and accepted the use of photographic documentation of the process in this work. 1. Enter/ Initiate The occupational therapy process with Tadeusz was initiated in 2011, when he was 46 years old. The client is married and has two adult sons. He lives in a detached house in Mociska in the suburbs of Warsaw. He was diagnosed with MS three years before the study, at the age of 43. However, two years prior to the diagnosis he had noted certain symptoms of MS. It was then that he fell down on an even surface for the first time. Soon afterwards he fell down again. A few months after the second collapse he noticed that his left leg trembled while pressing the clutch. Afterwards, similar tremors repeated occasionally. Finally, he decided to make an appointment with a surgeon to explain his ailments. He was referred to electroneurographic examination, yet the test showed no disorders. Jeszcze póniej podobne drenia powtarzaly si, aczkolwiek sporadycznie. W kocu pan Tadeusz zdecydowal si pój do lekarza- chirurga, w celu ustalenia przyczyny swoich dolegliwoci. Lekarz zlecil badanie przewodzenia nerwów obwodowych, ale otrzymany wynik badania byl poprawny. W maju 2007 r. mczyzna poddal si kolejnym badaniom. Panu Tadeuszowi wykonano rezonans magnetyczny i punkcj krgoslupa. Tym razem wyniki bada nie byly poprawne, a lekarz jednoznacznie zdiagnozowal u klienta stwardnienie rozsiane (SM), posta pierwotnie postpujc [8]. W tamtym okresie pan Tadeusz byl leczony w szpitalu, na Oddziale Neurologicznym, z powodu narastajcego od okolo roku oslabienia sily lewej koczyny dolnej i bólów odczuwanych w odcinku szyjnym i ldwiowym krgoslupa. W badaniu neurologicznym stwierdzono niedowlad niewielkiego stopnia lewej koczyny górnej i niedowlad lewej koczyny dolnej. Badanie MR glowy ujawnilo liczne, drobne ogniska demielinizacyjne w obu pólkulach mózgu oraz ciele modzelowatym. W karcie informacyjnej leczenia szpitalnego opisujcej histori choroby stwierdzono u pacjenta: ­ niedowlad trzykoczynowy wieloprzyczynowy; ­ rozsiane demielinizacyjne uszkodzenie mózgu; ­ wielopoziomow dyskopati krgoslupa w odcinku C, Th, L-S z uszkodzeniem rdzenia krgowego w odcinku szyjnym krgoslupa (ródlo: Karta leczenia szpitalnego udostpniona do wykorzystania na potrzeby niniejszego artykulu). Pana Tadeusza poddano leczeniu Solu- Medrolem (0,5g), w trakcie którego stan chorego ulegl poprawie. Ustpily dolegliwoci bólowe. Zaobserwowano ustpienie niedowladu lewej koczyny górnej oraz zmniejszenie niedowladu lewej koczyny dolnej. Pan Tadeusz jest rencist, posiadajcym orzeczenie o niepelnosprawnoci. Potrzebuje stalej opieki i pomocy innej osoby w zwizku ze znacznie ograniczon moliwoci samodzielnej egzystencji. Potrzebuje równie wspólpracy i pomocy w procesie leczenia, rehabilitacji i edukacji. Mczyzna porusza si przy pomocy kul inwalidzkich lub korzysta z wózka inwalidzkiego. Jest pacjentem wiadomym ­ duo czyta na temat swojej choroby i duo ju o niej wie, co pomaga mu wspólpracowa z lekarzami i fizjoterapeutami oraz wiadomie uczestniczy w procesie terapeutycznym. Magnetic Resonance (MR) and spine puncture. This time the results undoubtedly indicated the primary progressive type of MS [8]. At the time, due to the weakening of his left leg and pains in cervical and lumbar spine, he was admitted to hospital and treated in neurology ward. The neurological examination revealed a slight paresis of the left upper limb and a paresis of the left lower limb. The MR of the head revealed numerous demyelination foci in both brain hemispheres and corpus callosum. The client's medical history revealed that he suffered from: ­ triparesis; ­ disseminated encephalomyelitis; ­ cervical (C), thoracic (Th) and lumbosacral (LS) discopathy with a spinal cord injury in the cervical segment of the spine (source: hospital records made available with the client's consent to use this information in the article). The client underwent treatment with Solu-Medrol (0.5 g), as a result of which his condition improved and the pain disappeared. The paresis in the left upper limb disappeared, while the paresis in the left lower limb subsided. The client receives health benefits on the basis of his disability certificate. He needs constant care and help of another person due to his limited independence. He also needs everyday cooperation in the process of treatment, rehabilitation and education. He moves around using crutches or a wheelchair. He is a fully aware person, he reads a lot about his disease and this knowledge helps him cooperate with doctors and physiotherapists, and participate in the therapeutic process consciously. In the case of MS clients, occupational therapy supplements other forms of therapy and the fact that it sets a clear goal for a client is its advantage. Therefore, occupational therapy exerts influence on physical, neurophysiological and psychotherapeutic aspects [9]. The inclusion of the client in the process of occupational therapy whose aim was to improve the disturbed functions supplemented and supported the implemented physiotherapy. Moreover, the participation in occupational therapy is aimed at improving physical and functional fitness indispensable during self-care activities. An effective performance of a task or duty is to make the client aware of his capabilities, restore his mental balance and motivate him to work more. Udzial w terapii zajciowej ma ponadto na celu popraw sprawnoci fizycznej i funkcjonalnej niezbdnej podczas wykonywania czynnoci samoobslugowych. Efektywne wykonanie podjtego zajcia lub pracy ma uwiadomi Klientowi jego moliwoci, przywróci równowag psychiczn i zmotywowa do dalszej pracy. Zawsze jednak naley pamita o tym, aby planujc zajcia stawia klientowi taki cel, który w danym momencie jest on w stanie osign [10]. 2. Identyfikacja problemu klienta Ustalenie trudnoci, z którymi boryka si Klient nastpilo w wyniku wywiadu z nim samym i jego on, a take podczas obserwacji jego sposobu poruszania si i zapoznania si z treci dokumentacji medycznej. 2. Set the stage The obstacles which the client encountered were determined during an interview with him and his wife as well as while observing his manner of moving and after getting to know the hospital records. Typical daily activities were recognised and his expectations, needs and fears connected with everyday activities were defined. It appeared that the man had problems with keeping balance (he claimed to lose his balance often), which brought about his fear of falling. He would like to be independent for as long as possible (he is afraid of being dependent on his wife e.g. while preparing meals, eating, taking a shower or getting dressed). It was revealed that the muscles in his left arm and leg were weaker than in the right arm and leg (grip strength in the right and left hand of the client was examined by squeezing the therapist's hand). The client used crutches when moving around his house. However, he used a wheelchair on longer distances, e.g. when going to a doctor or meeting friends. He was able to drive a car with automatic transmission. 3. Pan Tadeusz poinformowal, e po terenie wlasnego domu porusza si przy pomocy kul inwalidzkich, ale poza domem, wówczas, gdy musi pokona dluszy dystans korzysta z wózka inwalidzkiego (np. gdy wyjeda na wizyt do lekarza lub na spotkania ze znajomymi). Jest w stanie prowadzi samochód wyposaony w automatyczn skrzyni biegów. 3. Ewaluacja ­ ustalenie potrzeb klienta Podczas wywiadu z klientem, dla zidentyfikowania jego problemów i potrzeb zastosowano uywany powszechnie w takim przypadku w terapii zajciowej kwestionariusz wywiadu COPM (Canadian Occupational Performance Measure) (uzyskano zgod na wykorzystanie narzdzia). He was asked to present difficulties that he encountered when performing various tasks, as well as his needs and expectations concerning an improvement in his independence. Occupational difficulties and problems were identified with the use of COPM questionnaire prepared by M. Law et al. [11]. COPM is an individualised questionnaire used by occupational therapists in order to define changes in selfassessment of occupational skills occurring in a particular period. The results of the interview carried out using COPM questionnaire are presented in the table below. Therapeutic goals: Planning and implementing an adaptation of a home bathroom to the needs of an MS client, supporting the client's process of learning to use it and gaining independence when taking a shower. The client's self-assessment connected with the ability to perform a particular activity/task and the level of satisfaction (tab. 2) is presented in steps 3 and 4. Both initial assessment (before the therapy) and reassessment (after adaptation) are illustrated in the table. The therapist confirmed 4 or 5 important problems with the client and recorded them in the questionnaire. Then, the therapist asked the client to rate each problem with regard to the performance and satisfaction and, afterwards, the total scores were recorded in the questionnaire. Identyfikacja trudnoci i problemów zwizanych z wykonywaniem przez mczyzn rónych zaj nastpila w oparciu o tre kwestionariusza COPM opracowanego przez M. Law i wspól. [11]. COPM jest zindywidualizowanym formularzem wykorzystywanym przez terapeutów zajciowych w celu ustalania zmian w samoocenie umiejtnoci zajciowych wystpujcych na przestrzeni okrelonego czasu. Wyniki wywiadu z wykorzystaniem kwestionariusza COPM prezentuje tabela 1. Kroki 3 i 4 ilustruj samoocen Klienta, która jest zwizana z ocen moliwoci wykonania danego zajcia/czynnoci i wynikajcym z tego poziomem zadowolenia (tab.2). Dotycz oceny wstpnej (przed usprawnianiem) i ponownej (po zastosowaniu adaptacji). Terapeuta ustala wspólnie z klientem 45 takich czynnoci, które sprawiaj mu najwikszy problem i zapisuje je w arkuszu kwestionariusza. Nastpnie prosi klienta o ocenienie kadej czynnoci/zajcia pod wzgldem moliwoci jej wykonania i satysfakcji. Wyniki samooceny zapisuje w kwestionariuszu. Wynik lczny oblicza si dodajc oceny za wykonanie lub satysfakcj za wszystkie trudnoci i podzielenie otrzymanej liczby przez liczb trudnoci (oblicza si w ten sposób warto redni). Na tym etapie ustala si z klientem t czynno/zajcie, które sprawia mu najwicej trudnoci i potrzebuje j usprawni. W ocenie ponownej klient ocenia t czynno, która zostala wskazana przez niego do poprawy. Ocena dotyczy moliwoci wykonania i wynikajcej z tego satysfakcji. Terapeuta ponownie zapisuje wyniki oceny w kwestionariuszu (krok 4) i podaje warto wzrostu oceny (rónica punktów pomidzy drugim i pierwszym badaniem) [12]. W wyniku przeprowadzonego wywiadu i oceny wstpnej ustalono z klientem glówny problem. Pan Tadeusz zdecydowal, e potrzebuje by bardziej samodzielny podczas kpieli. W zwizku z tak okrelon were calculated by adding together the performance or satisfaction scores for all the problems and dividing the sum by the number of problems (it provides a mean value). At this stage, the most difficult activity which needed improvement was identified. During the reassessment, the client rated the problem which was selected for improvement. The assessment regarded performance and satisfaction. The therapist recorded the score in the questionnaire again (step 4) and provided the value of an increase in the assessment (difference in points between the first and the second assessment) [12]. As a result of the interview and initial assessment, the main problem was identified together with the client. He decided that he needed to be more independent while bathing. Taking into account the client's main problem, we formulated hypotheses which expressed a probable reason for the problem, i.e. the lack of possibility to take a shower independently (without the help from other people). It was concluded that the problem may result from the fear of losing balance and falling down in the bathroom, weakness in the left hand muscles or the lack of the bathroom rails adjusted to the needs of a person with MS. 4. Agree on objectives/ treatment plan The next step involved discussing and taking a decision concerning the range of adaptations in the bathroom. The client's bathroom before the adaptation is presented below (fig. 1 and 2). The major difficulties which the client encountered in everyday life were discussed with him. It turned out that being independent while bathing was the most problematic and he wanted to take a shower without the help of his wife. During that therapeutic Ryc. 1. i 2. The bathroom before the adaptation potrzeb sformulowano hipotezy, które wyraaj prawdopodobn przyczyn wskazanego problemu, jakim jest brak moliwoci samodzielnego (bez pomocy innych osób) brania kpieli. Uznano, e problem moe wynika z: obawy o utrat równowagi i moliwo upadku w lazience; oslabienia sily mini lewej rki; braku odpowiednich urzdze (adaptacji) w lazience, które odpowiadalyby potrzebom osoby chorujcej na SM. 4. Uzgodnienie celu i planu interwencji Nastpnym krokiem bylo przedyskutowanie i zdecydowanie z Klientem o zakresie wykonania adaptacji w jego domowej lazience. Na poniszych fotografiach zaprezentowano lazienk w domu Klienta przed wykonaniem ustalonego przystosowania (ryc. 1 i 2). Omówiono z Klientem te trudnoci, których dowiadcza podczas dnia codziennego. Okazalo si, e najwicej problemu sprawia Panu Tadeuszowi samodzielna kpiel. Mczyzna owiadczyl, e chcialby session the range of indispensable changes (adaptations) in the bathroom which would meet the expectations and needs of the client and the way in which the client would learn to use them were agreed on. The ongoing evaluation of independence while bathing confirmed big difficulties in performing this task (the result of the initial assessment made by the client: performance ­ 4; satisfaction ­ 4). The agreed adaptation of the bathroom to the client's needs included making a wooden step covered with an anti-slip mat (an easier way to go into the bath) (fig.3) and handrails fixed to both walls (making it easier to keep balance and protecting from a potential fall). The occupational therapist's task performed in cooperation with the client was to make him feel safe in his bathroom and be self-reliant while having a bath. Podczas tej sesji terapeutycznej ustalono zakres niezbdnych do wykonania zmian (adaptacji) w lazience, które odpowiadalyby oczekiwaniom i potrzebom Klienta oraz sposób w jaki mczyzna bdzie si uczyl z nich korzysta. Ocena bieca ­ samodzielnoci i niezalenoci podczas kpieli ­ potwierdza, e Klient dowiadcza duych trudnoci z wykonaniem tej czynnoci (wynik oceny wstpnej Klienta: wykonanie ­ 4; satysfakcja ­ 4). Ustalony zakres dostosowania lazienki do potrzeb Klienta uwzgldnia: wykonanie drewnianego stopnia pokrytego mat antypolizgow (ulatwienie wejcia do wanny) (ryc.3) i porczy zamontowanych na dwóch cianach lazienki (umoliwi zachowanie równowagi i ochroni przed ewentualnym upadkiem). Zadaniem terapeuty zajciowego we wspólpracy z Klientem jest spowodowanie tego, aby mczyzna mógl czu si bezpiecznie we wlasnej lazience i byl samowystarczalny podczas codziennej kpieli. This method makes it possible to describe the goals of occupational therapy clearly in the following categories: S (significant) ­ describing the patient, M (measurable) ­ defining the tasks in the therapy, A (achievable) ­ defining the period during which the results should be achieved, R (realistic) ­ indicating activities planned within the occupational therapy with regard to the previously set goal, T (timed) ­ determining the time of working with the client until the reassessment of the results. T ­ two weeks During the second therapeutic session with the client on January 4th, 2011, the following steps were taken: ­ discussing the concept of adapting the bathroom to the client's needs which were previously reported; ­ providing the client with details on the adaptation, including the information where and how the equipment will be fixed; ­ taking measurements indispensable for manufacturing the step and rails (length, height, width); ­ establishing together with the client where exactly the rails should be fixed; ­ obtaining the client's and his wife's consent to realise the presented plan and implementing it. 5. Implement the plan The third visit at the client's house took place on January 29th, 2011. During that session the bathroom was adapted according to the previous plan. Handrails and a step were mounted (fig.4). Moreover, the manner of using the equipment was explained and practised (fig.5). Ryc. 4. Bathroom adaptation (handrails and wooden step covered with an anti-slip mat) Podczas drugiej sesji z Klientem, w dniu 4 stycznia 2011 r. podjto nastpujce dzialania: ­ przedyskutowano koncepcj wprowadzenia w lazience niezbdnych udogodnie w zwizku ze zgloszonymi uprzednio potrzebami; ­ poinformowano o tym, jak bdzie wyglda zaproponowana adaptacja, zademonstrowano w jaki sposób i gdzie zostanie zamontowana; ­ wykonano pomiary niezbdne do wykonania stopnia i porczy (dlugo, wysoko, szeroko); ­ ustalono wspólnie z Klientem dokladne miejsca zamontowania porczy; ­ uzyskano zgod Klienta i jego ony na realizacj przedstawionego planu i wykonanie adaptacji w lazience. 5. Wdroenie/wykonanie planu Trzecia wizyta domowa u Klienta miala miejsce w dniu 29 stycznia 2011 r. Tego dnia zostaly wprowadzone w lazience ustalone zmiany. Zamontowano porcze i stopie umoliwiajcy wchodzenie do wan- The analysis below shows particular stages of learning the activity according to the Taxonomic Code of Occupational Performance [13]. 1. Voluntary movement or mental processes: exercising lower limb, arm and trunk muscles ­ flexion, extension, adduction, abduction, rotation; mental processes ­ focusing on the activity, remembering successive steps. 2. Action: grabbing and holding the rail, walking with and without crutches, ascending the step, getting into the bathtub, maintaining a sitting position in the bathtub, rotating the body; thinking constantly about performing successive stages of the activity. 3. Task: entering the bathroom, leaving crutches on the left side, grabbing the rail, standing and maintaining balance, getting into the bathtub, undressing, getting dressed, using a towel and bath accessories, learning, thinking, solving problems, using the right hand, following instructions such as `go towards the step', `grab the rail with two hands', `put your right foot on the step', `put your Ryc. 5. Activity: remembering the performed activities, thinking according to a proper order, bathing without the help of others. 5. Occupation: walking, standing, grabbing, putting, entering the bathtub, getting dressed, undressing, using the towel and bath accessories, learning new activities and behaviours, thinking analytically, solving problems. 6. Monitor and modify It was agreed that the client would be contacted by phone and, if necessary, would be visited at home. At the stage when the client tested the implemented solutions, his progress and potential difficulties were monitored and discussed both with him and with his wife. It was checked whether the new devices were helpful in everyday bathing. The client's wife stated that during the process of learning how to bathe independently, he used the implemented equipment trying to remember successive stages of this activity. He rarely needed hints concerning what he should do and how at that particular moment. He learned how to perform the activity in the correct order, which helped him become more and more independent. He practised grabbing, moving without crutches and going up and down the step located next to the bathtub. At that time the client also practised regularly with his physiotherapist (strengthening muscles) and performed the recommended physical exercises by himself. After a few cases of getting into the bathtub with the use of the step, he decided that he did not need the anti-slip mat since he felt confident without it while standing on the step. Upon the request of the client, the mat was removed and it was the only modification to the previously established therapy plan. 7. Evaluate outcome The fourth meeting with the client took place on February 14th, 2011. Reassessment (using COPM) was performed two weeks after the implementation of the supporting devices. The client and his wife confirmed that the previously defined tasks (bathing independently) were realized and the goals were being gradually achieved. The assessment of performance and satisfaction increased by 3 points (tab. 2). The client said: I am very happy with the equipment that has been installed in my bathroom. Po wykonaniu kilku prób wejcia do wanny z uyciem stopnia, mczyzna uznal, e na t chwil nie potrzebuje maty antypolizgowej, poniewa czuje si pewnie na stopniu nawet bez pokrycia mat. Na wniosek Klienta zrezygnowano z uycia maty, i byla to jedyna modyfikacja jak wprowadzono do przyjtego planu terapii. 7. Ocena wyników terapii Czwarta sesja z Panem Tadeuszem odbyla si w dniu 14 lutego 2011 r. Powtórn ocen z wykorzystaniem kwestionariusza COPM przeprowadzono po uplywie dwóch tygodni od wprowadzenia ustalonej adaptacji w lazience Klienta. Uzyskano zapewnienie od mczyzny i jego ony, e okrelone uprzednio zadania (samodzielna kpiel bez pomocy) i cele s stopniowo osigane. Ocena wykonania zadania i satysfakcji na tym etapie terapii wzrosla o 3 punkty (tab.2). Klient powiedzial: ,,Jestem bardzo zadowolony z urzdze, które zostaly zamontowane w mojej lazience. W tej chwili nie obawiam si o to, e mog upa. Wiem, e kiedy bd tracil równowag, to mog chwyci si porczy. Czuj si z tym lepiej i jestem bardziej pewny siebie. Jestem bardziej spokojny i usatysfakcjonowany z tego powodu, e mog wykona codzienn toalet, poniewa jestem bardziej samodzielny podczas kpieli, golenia si i mycia. Stopie jest dobrze dopasowany i ulatwia mi wejcie i wyjcie z wanny. Porcz pod oknem umoliwia korzystanie z toalety (latwiej mi wstawa z sedesu). To byl dobry projekt, a wprowadzone w mojej lazience zmiany uwaam za funkcjonalne. S to "meble" pierwszej potrzeby, które pozwalaj mi bardziej ufa sobie samemu. Niczego lepszego w tej chwili nie potrzebuj". ona Pana Tadeusza uwaa, e ,,m jest teraz bardziej pewny siebie. Wane jest to, e kiedy porusza si w lazience moe zawsze chwyci si porczy, kiedy tylko tego potrzebuje. Mój m stal si bardziej samodzielny w kpieli. Teraz rzadziej mu pomagam. W te dni, kiedy lepiej si czuje w ogóle moe kpa si samodzielnie. Wczeniej zawsze musialam mu pomaga. Jestem wprawdzie obecna w lazience, ale wylcznie go obserwuj i czasami tylko podpowiadam, co powinien zrobi. Kiedy m gorzej si czuje, pomagam mu postawi lew nog na stopniu. Jestem zadowolona z tych urzdze, które uwaam za este- present I am not afraid that I will fall down. I know that if I lose balance, I can grab the rail. I feel better and more confident. I'm more relaxed and satisfied with the fact that I can wash myself because I am more independent while bathing, shaving and washing. The step fits well and helps me to get in and out of the bathtub. Also, the grab rail under the window makes it easier to use the toilet (I can easily stand up from the toilet seat). It was a good idea and I think the changes implemented in my bathroom are functional. This is necessary "furniture" which allows me to be more self-assured. I don't need anything better. His wife stated: "My husband is more confident now. It is important for him that when he is moving in the bathroom, he may grab the rail whenever he needs it. My husband is more independent while bathing. Now, I help him less frequently. When my husband has a good day and feels better, he is able to bathe himself. Previously, I always had to help him. Now I'm present in the bathroom, but I'm just watching him, and sometimes I tell him what to do. When my husband feels worse, I help him put his left foot on the step. I'm happy with the equipment which is aesthetic. Also, the rails have one more function ­ they serve as towel rails. I am glad that my husband is more independent while bathing. Conclusion The client is aware of the fact that his disease is progressing. He understands that with time he will become less physically fit. However, he confirmed that he was open to further cooperation with the occupational therapist and physiotherapist in order to maintain the level of fitness and independence. In the two-week period, the patient explored new possibilities and engaged in learning how to use newly fixed equipment. It was noted that the client was constantly looking for ways to improve his level of independence and perform tasks connected with using the bathroom better. Moreover, he regularly participated in the therapy with his physiotherapist and performed exercises at home. The man constantly looks for information concerning his disease and talks to other MS patients on the Internet (Facebook, blogs, etc.). He often discusses the disease with them in order to gain more information and help others understand it better and accept it. He is also a member of the Polish Multiple Sclerosis Society. The sessions with the client were also valuable due to the fact that issues other than those associated with the disease could be discussed with him, which was also significant for the client tycznie wykonane. Potwierdza jednak, e jest otwarty na dalsz wspólprac z terapeut zajciowym i fizjoterapeut, w celu podtrzymywania sprawnoci i samodzielnoci. Podczas tych dwóch tygodni przed Panem Tadeuszem otworzyly si nowe moliwoci i z duym zaangaowaniem uczyl si korzysta z zainstalowanych w lazience urzdze. Zaobserwowano, e Klient wci szukal sposobów na to, aby podnie na wyszy poziom swoj niezaleno i lepiej wykonywa zajcia zwizane z korzystaniem z lazienki. Ponadto regularnie uczestniczyl w terapii ze swoim fizjoterapeut, a take codziennie wykonywal zestaw wicze gimnastycznych w domu. Mczyzna stale poszukuje informacji o swojej chorobie, rozmawia z innymi osobami, które take cierpi z powodu SM, a czyni to za porednictwem Internetu (facebook, blogi itp.). Czsto dyskutuje z nimi na temat choroby, po to, aby wicej si na jej temat dowiedzie, ale te innym pomóc j lepiej pozna i zaakceptowa. Pan Tadeusz jest te czlonkiem Polskiego Towarzystwa Stwardnienia Rozsianego. W sesjach odbytych z Panem Tadeuszem wartociowe bylo te to, e mona bylo porozmawia z Klientem równie na inne tematy ni tylko te zwizane z chorob, co ­ jak zauwaono ­ bylo dla mczyzny bardzo wane. Podczas wspólpracy z Panem Tadeuszem zaobserwowano, e jest to pogodny mczyzna. Nawet kiedy odczuwa ból stara si umiecha i artowa. Ma wewntrzn sil do walki z chorob i wynikajcymi z niej ograniczeniami, chtnie te mówi o swojej chorobie. Kiedy powiedzial, e chce by niezaleny i samodzielny w zakresie codziennych zaj, tak dlugo, jak to tylko bdzie moliwe. A z tego powodu, e nie chcialby porusza si wylcznie na wózku inwalidzkim, to codziennie wykonuje swoje wiczenia, wzmacniajc minie nóg i rk. Jest wspierany przez swoich najbliszych ­ on i synów, którzy rozumiej trudnoci, których dowiadcza w codziennym yciu i pomagaj mu kiedy tylko tego potrzebuje. Zrealizowany z Panem Tadeuszem proces terapii zajciowej byl bardzo satysfakcjonujcy. It was also noted that the client is a cheerful person. Even though he felt pain, he was trying to smile and joke. He has internal strength to fight the disease and limitations resulting from it, and is willing to talk about the disease. He once said that he wanted to be independent and self-reliant in everyday life for the longest time possible. Every day, he performs leg and arm muscle strengthening exercises in order not to be dependent on a wheelchair only. His wife and sons support him as they understand difficulties which he experiences in everyday life and help him whenever he needs it. The process of occupational therapy realised with the client was really satisfactory. It was significant that the help provided to the client enabled him to perform everyday activities more efficiently and was adequate to his needs. Success and achievement of the goals of occupational therapy depends on a few factors, one of them being conscious and active engagement of the client. His openness and trust developed towards the occupational therapist were a starting point for good cooperation and exerted significant influence on the efficiency and effectiveness of the process. In a short period from the bathroom adaptation, the client was able to note and experience an improvement in his own performance and independence. The study was conducted within the project realised at the University of Physical Education in Warsaw titled "The improvement of didactic and scientific potential in the field of occupational therapy as a key to the development of higher education" co-financed by the European Union within the European Social Fund. Abstract Introduction: Multiple sclerosis is a disease attacking the central nervous system. It develops when the myelin sheaths of nerve fibres, glial cells and immune cells are damaged [3]. The aim of this study was to present the project of adapting a bathroom to the needs and expectations of a person with MS and to show its implementation. Method: A direct interview with an MS client was conducted with COPM questionnaire (often used in occupational therapy) and medical documentation was analysed. Case study: Tadeusz (the client) was a 46-year-old man. He was diagnosed with MS when he was 43 years old. However, the first symptoms occurred two years earlier. In 2007 he underwent magnetic resonance and spine puncture examinations. The results confirmed a primary progressive type of MS. Identification of the client's problem: the need of independence while washing, shaving and bathing. Tadeusz was not able to take a shower by himself because of the fear of losing balance and falling down in the bathroom. The weakness of the left hand muscles and the lack of support rails made it impossible to use the bathroom independently. During several meetings with the client, a plan of adapting his bathroom (fixing handrails and a wooden step) was developed and the manner of using these devices was practised. Conclusions: Two weeks after the bathroom adaptation was finished, it was assessed together with the client. It was concluded that the adaptation meets his expectations and the aim is being gradually achieved. In the reassessment process, an improvement by 3 points was noted. The "task performance" and "satisfaction" criteria were given 7 points (in the scale from 1 to 10) by the client. However, in the first assessment he evaluated these criteria by giving them 4 points each. Naley do grupy tzw. chorób autoimmunologicznych. Jest to choroba nieuleczalna. Nie uwaa si jednak, e zachorowanie na stwardnienie rozsiane skraca ycie, aczkolwiek choroba ta znaczco obnia jego komfort i jako [2]. Stwardnienie rozsiane jest chorob, która dotyczy komórek nerwowych (neuronów), komórek glejowych (oligodendrocyty) i odpornociowych mózgu, w których na skutek toczcego si procesu chorobowego zostaje uszkodzona oslona mielinowa. W rezultacie powoduje to nieprawidlowy przeplyw impulsu nerwowego pomidzy komórkami nerwowymi mózgu i rdzenia krgowego [3]. SM jest przewlekl, nieuleczaln chorob centralnego ukladu nerwowego, w wyniku której niszczona jest tkanka nerwowa. Po raz pierwszy chorob opisal francuski neurolog Jeana Martina Charcot, w 1868 roku. Jej nazwa podkrela to, e procesy patologiczne zwizane z chorob s rozsiane w rónych obszarach ukladu nerwowego, jak równie rozsiane w czasie. Badacz wskazal na trzy glówne symptomy choroby, tzn. dyzartri (zaburzenie mowy, charakteryzujce si tym, e staje si ona powolna i niewyrana), ataksj (zaburzenie koordynacji ruchowej) i drenie mini. Opisal równie zmiany Multiple sclerosis is a rare disease, i.e. a disease that occurs in 5 per 10,000 cases [1]. It is an autoimmune and incurable disease. However, it does not shorten life but significantly reduces its comfort and quality [2]. Multiple sclerosis is a disease affecting neurons (neuronal cells), glial and immune cells whose myelin sheaths have been damaged. It results in improper passing of neuron impulse throughout brain cells and spinal cord [3]. It is a chronic, incurable disease of the central nervous system in the course of which nervous tissue gets damaged. It was first described by a French neurologist Jean Martin Charcot in 1868. Its name points to the fact that pathological processes connected with the disease appear in different parts of the nervous system. The symptoms are also scattered in time. The researcher indicated three main symptoms, i.e. dysarthria (speech disorder manifesting itself in slow and unclear articulation), ataxia (physical coordination disorder) and muscular tremor. He also described cognitive disturbances in MS patients, i.e. a significant "loss of memory, slowed down thinking and learning difficulties" [4]. Styl ycia bynajmniej nie ma wplywu na rozwój tej choroby. Nie ma te jednoznacznych dowodów na to, e jest to choroba dziedziczna, aczkolwiek ostatnie wyniki bada dowodz, e chorob wywoluje kombinacja czynników genetycznych i rodowiskowych. Z cal pewnoci mona jednak stwierdzi, e stwardnienie rozsiane nie jest chorob zakan [5]. W literaturze wskazuje si na cztery typy tej choroby: posta remitujco- nawracajc, wtórnie postpujc, pierwotnie postpujc i posta postpujconawracajc [5]. SM jest chorob, która dotyka przede wszystkim ludzi mlodych, midzy 20. a 40. rokiem ycia, ale zdarza si te u osób poniej 18. roku ycia, a nawet u dzieci; 6. prawdopodobiestwo wystpienia SM u osób mieszkajcych dalej od równika jest wiksze (dowodem na to jest ogromna liczba zachorowa w krajach skandynawskich). Aktualnie, nie ma jeszcze leków, które moglyby skutecznie leczy SM, ale s takie, które hamuj rozwój tej choroby. S to leki immunomodulujce (np. interferon), które modyfikuj prac systemu immunologicznego. Terapia lekami znaczco obnia liczb ataków i zaostrze w przebiegu choroby. Interferon jest lekiem bardzo skutecznym, ale równie kosztownym, i dlatego w Polsce nie wszyscy mog mie dostp do takiego leczenia. Aby otrzyma refundacj kuracji interferonem, polski pacjent musi zosta zakwalifikowany do leczenia przez Narodowy Fundusz Zdrowia (NFZ), i spelnia okrelone kryteria: wiek poniej 35. r. .; czas trwania choroby; dotychczasowa (numbness or tingling sensation in arms and legs), visual disorders (unclear and double vision), pain, cognition and mood disturbances, chronic fatigue (general weakness that render physical and intellectual activity impossible), muscle fatigue and spasticity or problems with digestive and urinary systems [4]. The development of this disease is by no means influenced by lifestyle. Nor is there any explicit evidence for its genetic causes. However, the latest research has proved that it is caused by a combination of genetic and environmental factors. It can certainly be concluded that it is not a contagious disease [5]. In the literature of the subject there occur four types of this disease, i.e. relapsing-remitting MS, secondary progressive MS, primary progressive MS and progressive-relapsing MS [5]. Statistics concerning MS are as follows [2,5]: 1. there are approximately 2.5 million people with MS in the world; 2. there are approximately 500,000 people with MS in Europe; 3. there are approximately 40,000 people with MS in Poland (one case per one thousand healthy individuals); 4. ca. 75% of those who have MS in Poland start receiving health benefits within two years from the diagnosis; 5. MS is most often diagnosed between the age of 20 and 40 but it may also occur in teenagers below 18 or even among children; 6. the risk of developing MS rises with the increasing distance from the equator (illustrated by an immense number of MS cases in Scandinavian countries). Currently, there is no cure for MS but there are drugs which can slow down the development of the disease. They are called immunomodulatory drugs (e.g. interferon) as they modify the work of the immune system. Drug treatment significantly reduces the number of attacks and exacerbations of the disease. Interferon is a very effective yet costly drug and, therefore, not all individuals with MS can gain access to such treatment in Poland. In order to receive free interferon treatment, a Polish patient has to be qualified by the Polish National Health Service (NHS) and meet certain criteria, such as age below 35, duration of the disease, the number of previous attacks or particular types of symptoms. In Poland, only ca. 8% of MS patients are treated with interferon funded by NHS, whereas in other European Union countries the number reaches approximately 30%. Thus, Poland is in the last but one position as far as accessibility to interferon therapy is concerned [5]. liczba rzutów choroby; rodzaj objawów. Stwardnieniu rozsianemu towarzyszy wiele objawów, które znaczco obniaj jako ycia chorego. Dlatego dobrze i systematycznie prowadzona rehabilitacja, cho nie ogranicza istotnie kolejnych rzutów choroby, ani te jej nie hamuje, to jednak poprawia zarówno obiektywne wskaniki zdrowia, jak i subiektywnie oceniane samopoczucie. Terapia zajciowa jako jeden z rodzajów rehabilitacji chorego na SM okazuje si mie ogromne znaczenie dla poprawy jego niezalenoci podczas wykonywania codziennych czynnoci, a take usprawniania funkcji poznawczych [6]. I, chocia nie okrela si jednoznacznych wskaza czy zalece co do najbardziej skutecznej formy terapii w przypadku osób cierpicych na stwardnienie rozsiane, to badania jednak potwierdzaj istotne rónice dotyczce stanu zdrowia u osób poddawanych terapii i nierehabilitowanych [6]. Celem niniejszego opracowania jest przedstawienie i opisanie autorskiego projektu na przystosowanie lazienki do oczekiwa i potrzeb osoby chorujcej na SM, zaprezentowanie jego wdroenia oraz omówienie procesu terapii zajciowej przeprowadzonego z klientem chorujcym na stwardnienie rozsiane. Opis przypadku MS is accompanied by numerous symptoms which significantly decrease the patients' quality of life. Thus, although a proper and regular rehabilitation does not limit the number of further attacks or inhibit the development of the disease, it improves both objective health indices and subjective well-being. Occupational therapy is one of the types of MS patient rehabilitation which seems to be really significant in the process of improving their independence during everyday activities and improving cognitive functions [6]. Despite the fact that there are no unanimous indications or recommendations regarding the most effective form of therapy for MS patients, the research confirmed significant differences in the state of health between patients who undergo rehabilitation and those who do not [6]. The aim of this study was to present and describe the project of adapting a bathroom to the needs and expectations of an MS client, to show its implementation process and to discuss the process of occupational therapy conducted with the MS client. Zainicjowanie kontaktu z klientem Wspólprac z panem Tadeuszem w zwizku z podjtym procesem terapii zajciowej nawizano w 2011 roku. Wówczas byl to 46-letni mczyzn. Pan Tadeusz jest onaty i ma dwóch doroslych synów. Mieszka z rodzin w domu jednorodzinnym, w miejscowoci Mociska poloonej w pobliu Warszawy. Cierpi na stwardnienie rozsiane, które zdiagnozowano u nie- The work presents a case study of a man with MS prepared on the basis of the Canadian Practice Process Framework (CPPF), which is widely used in the process of occupational therapy [7]. CPPF includes certain steps which an occupational therapist takes with a client during consecutive therapeutic sessions. The client gave his consent to participate in an interview and occupational therapy, and accepted the use of photographic documentation of the process in this work. 1. Enter/ Initiate The occupational therapy process with Tadeusz was initiated in 2011, when he was 46 years old. The client is married and has two adult sons. He lives in a detached house in Mociska in the suburbs of Warsaw. He was diagnosed with MS three years before the study, at the age of 43. However, two years prior to the diagnosis he had noted certain symptoms of MS. It was then that he fell down on an even surface for the first time. Soon afterwards he fell down again. A few months after the second collapse he noticed that his left leg trembled while pressing the clutch. Afterwards, similar tremors repeated occasionally. Finally, he decided to make an appointment with a surgeon to explain his ailments. He was referred to electroneurographic examination, yet the test showed no disorders. In May 2007 the man underwent additional examinations including go trzy lata wczeniej, w wieku 43 lat. Wkrótce po tym, przewrócil si ponownie. Po kilku miesicach od drugiego upadku, a zdarzylo si to jesieni, zauwayl, e podczas prowadzenia samochodu lewa noga zaczyna dre oparta o pedal sprzgla. Jeszcze póniej podobne drenia powtarzaly si, aczkolwiek sporadycznie. W kocu pan Tadeusz zdecydowal si pój do lekarza- chirurga, w celu ustalenia przyczyny swoich dolegliwoci. Lekarz zlecil badanie przewodzenia nerwów obwodowych, ale otrzymany wynik badania byl poprawny. W maju 2007 r. mczyzna poddal si kolejnym badaniom. Panu Tadeuszowi wykonano rezonans magnetyczny i punkcj krgoslupa. Tym razem wyniki bada nie byly poprawne, a lekarz jednoznacznie zdiagnozowal u klienta stwardnienie rozsiane (SM), posta pierwotnie postpujc [8]. W tamtym okresie pan Tadeusz byl leczony w szpitalu, na Oddziale Neurologicznym, z powodu narastajcego od okolo roku oslabienia sily lewej koczyny dolnej i bólów odczuwanych w odcinku szyjnym i ldwiowym krgoslupa. W badaniu neurologicznym stwierdzono niedowlad niewielkiego stopnia lewej koczyny górnej i niedowlad lewej koczyny dolnej. Badanie MR glowy ujawnilo liczne, drobne ogniska demielinizacyjne w obu pólkulach mózgu oraz ciele modzelowatym. W karcie informacyjnej leczenia szpitalnego opisujcej histori choroby stwierdzono u pacjenta: ­ niedowlad trzykoczynowy wieloprzyczynowy; ­ rozsiane demielinizacyjne uszkodzenie mózgu; ­ wielopoziomow dyskopati krgoslupa w odcinku C, Th, L-S z uszkodzeniem rdzenia krgowego w odcinku szyjnym krgoslupa (ródlo: Karta leczenia szpitalnego udostpniona do wykorzystania na potrzeby niniejszego artykulu). Pana Tadeusza poddano leczeniu Solu- Medrolem (0,5g), w trakcie którego stan chorego ulegl poprawie. Ustpily dolegliwoci bólowe. Zaobserwowano ustpienie niedowladu lewej koczyny górnej oraz zmniejszenie niedowladu lewej koczyny dolnej. Pan Tadeusz jest rencist, posiadajcym orzeczenie o niepelnosprawnoci. Potrzebuje stalej opieki i pomocy innej osoby w zwizku ze znacznie ograniczon moliwoci samodzielnej egzystencji. Potrzebuje równie wspólpracy i pomocy w procesie leczenia, rehabilitacji i edukacji. Mczyzna porusza si przy pomocy kul inwalidzkich lub korzysta z wózka inwalidzkiego. Jest pacjentem wiadomym ­ duo czyta na temat swojej choroby i duo ju o niej wie, co pomaga mu wspólpracowa z lekarzami i fizjoterapeutami oraz wiadomie uczestniczy w procesie terapeutycznym. Magnetic Resonance (MR) and spine puncture. This time the results undoubtedly indicated the primary progressive type of MS [8]. At the time, due to the weakening of his left leg and pains in cervical and lumbar spine, he was admitted to hospital and treated in neurology ward. The neurological examination revealed a slight paresis of the left upper limb and a paresis of the left lower limb. The MR of the head revealed numerous demyelination foci in both brain hemispheres and corpus callosum. The client's medical history revealed that he suffered from: ­ triparesis; ­ disseminated encephalomyelitis; ­ cervical (C), thoracic (Th) and lumbosacral (LS) discopathy with a spinal cord injury in the cervical segment of the spine (source: hospital records made available with the client's consent to use this information in the article). The client underwent treatment with Solu-Medrol (0.5 g), as a result of which his condition improved and the pain disappeared. The paresis in the left upper limb disappeared, while the paresis in the left lower limb subsided. The client receives health benefits on the basis of his disability certificate. He needs constant care and help of another person due to his limited independence. He also needs everyday cooperation in the process of treatment, rehabilitation and education. He moves around using crutches or a wheelchair. He is a fully aware person, he reads a lot about his disease and this knowledge helps him cooperate with doctors and physiotherapists, and participate in the therapeutic process consciously. In the case of MS clients, occupational therapy supplements other forms of therapy and the fact that it sets a clear goal for a client is its advantage. Therefore, occupational therapy exerts influence on physical, neurophysiological and psychotherapeutic aspects [9]. The inclusion of the client in the process of occupational therapy whose aim was to improve the disturbed functions supplemented and supported the implemented physiotherapy. Moreover, the participation in occupational therapy is aimed at improving physical and functional fitness indispensable during self-care activities. An effective performance of a task or duty is to make the client aware of his capabilities, restore his mental balance and motivate him to work more. Set the stage The obstacles which the client encountered were determined during an interview with him and his wife as well as while observing his manner of moving and after getting to know the hospital records. Typical daily activities were recognised and his expectations, needs and fears connected with everyday activities were defined. It appeared that the man had problems with keeping balance (he claimed to lose his balance often), which brought about his fear of falling. He would like to be independent for as long as possible (he is afraid of being dependent on his wife e.g. while preparing meals, eating, taking a shower or getting dressed). It was revealed that the muscles in his left arm and leg were weaker than in the right arm and leg (grip strength in the right and left hand of the client was examined by squeezing the therapist's hand). The client used crutches when moving around his house. However, he used a wheelchair on longer distances, e.g. when going to a doctor or meeting friends. He was able to drive a car with automatic transmission. 3. Assess/evaluate The Canadian Occupational Performance Measure (COPM), which is widely applied in occupa- Zapoznano si z czynnociami wykonywanymi przez Pana Tadeusza w cigu typowego dnia, ustalono jego podstawowe oczekiwania, potrzeby i obawy zwizane z zajciami i czynnociami codziennymi. Stwierdzil natomiast, e chcialby by samodzielny w zakresie poruszania si i samoobslugi tak dlugo, jak to tylko bdzie moliwe (nie chcialby by zaleny od ony np. podczas przygotowywania posilków, jedzenia, kpieli, czy ubierania si). Ustalono, e minie lewej rki i nogi s slabsze ni prawej (ustalono to podczas ciskania rki terapeuty, w celu sprawdzenia sily chwytu prawej i lewej dloni Klienta). Pan Tadeusz poinformowal, e po terenie wlasnego domu porusza si przy pomocy kul inwalidzkich, ale poza domem, wówczas, gdy musi pokona dluszy dystans korzysta z wózka inwalidzkiego (np. gdy wyjeda na wizyt do lekarza lub na spotkania ze znajomymi). Jest w stanie prowadzi samochód wyposaony w automatyczn skrzyni biegów. 3. Ewaluacja ­ ustalenie potrzeb klienta Podczas wywiadu z klientem, dla zidentyfikowania jego problemów i potrzeb zastosowano uywany powszechnie w takim przypadku w terapii zajciowej kwestionariusz wywiadu COPM (Canadian Occupational Performance Measure) (uzyskano zgod na wykorzystanie narzdzia). He was asked to present difficulties that he encountered when performing various tasks, as well as his needs and expectations concerning an improvement in his independence. Occupational difficulties and problems were identified with the use of COPM questionnaire prepared by M. Law et al. [11]. COPM is an individualised questionnaire used by occupational therapists in order to define changes in selfassessment of occupational skills occurring in a particular period. The results of the interview carried out using COPM questionnaire are presented in the table below. Therapeutic goals: Planning and implementing an adaptation of a home bathroom to the needs of an MS client, supporting the client's process of learning to use it and gaining independence when taking a shower. The client's self-assessment connected with the ability to perform a particular activity/task and the level of satisfaction (tab. 2) is presented in steps 3 and 4. Both initial assessment (before the therapy) and reassessment (after adaptation) are illustrated in the table. The therapist confirmed 4 or 5 important problems with the client and recorded them in the questionnaire. Then, the therapist asked the client to rate each problem with regard to the performance and satisfaction and, afterwards, the total scores were recorded in the questionnaire. COPM jest zindywidualizowanym formularzem wykorzystywanym przez terapeutów zajciowych w celu ustalania zmian w samoocenie umiejtnoci zajciowych wystpujcych na przestrzeni okrelonego czasu. Wyniki wywiadu z wykorzystaniem kwestionariusza COPM prezentuje tabela 1. Kroki 3 i 4 ilustruj samoocen Klienta, która jest zwizana z ocen moliwoci wykonania danego zajcia/czynnoci i wynikajcym z tego poziomem zadowolenia (tab.2). Dotycz oceny wstpnej (przed usprawnianiem) i ponownej (po zastosowaniu adaptacji). Terapeuta ustala wspólnie z klientem 45 takich czynnoci, które sprawiaj mu najwikszy problem i zapisuje je w arkuszu kwestionariusza. Nastpnie prosi klienta o ocenienie kadej czynnoci/zajcia pod wzgldem moliwoci jej wykonania i satysfakcji. Wyniki samooceny zapisuje w kwestionariuszu. Wynik lczny oblicza si dodajc oceny za wykonanie lub satysfakcj za wszystkie trudnoci i podzielenie otrzymanej liczby przez liczb trudnoci (oblicza si w ten sposób warto redni). Na tym etapie ustala si z klientem t czynno/zajcie, które sprawia mu najwicej trudnoci i potrzebuje j usprawni. Terapeuta ponownie zapisuje wyniki oceny w kwestionariuszu (krok 4) i podaje warto wzrostu oceny (rónica punktów pomidzy drugim i pierwszym badaniem) [12]. W zwizku z tak okrelon were calculated by adding together the performance or satisfaction scores for all the problems and dividing the sum by the number of problems (it provides a mean value). At this stage, the most difficult activity which needed improvement was identified. During the reassessment, the client rated the problem which was selected for improvement. The assessment regarded performance and satisfaction. The therapist recorded the score in the questionnaire again (step 4) and provided the value of an increase in the assessment (difference in points between the first and the second assessment) [12]. As a result of the interview and initial assessment, the main problem was identified together with the client. He decided that he needed to be more independent while bathing. Taking into account the client's main problem, we formulated hypotheses which expressed a probable reason for the problem, i.e. the lack of possibility to take a shower independently (without the help from other people). It was concluded that the problem may result from the fear of losing balance and falling down in the bathroom, weakness in the left hand muscles or the lack of the bathroom rails adjusted to the needs of a person with MS. 4. Agree on objectives/ treatment plan The next step involved discussing and taking a decision concerning the range of adaptations in the bathroom. The client's bathroom before the adaptation is presented below (fig. 1 and 2). The major difficulties which the client encountered in everyday life were discussed with him. It turned out that being independent while bathing was the most problematic and he wanted to take a shower without the help of his wife. During that therapeutic Ryc. 1. i 2. Uzgodnienie celu i planu interwencji Nastpnym krokiem bylo przedyskutowanie i zdecydowanie z Klientem o zakresie wykonania adaptacji w jego domowej lazience. Na poniszych fotografiach zaprezentowano lazienk w domu Klienta przed wykonaniem ustalonego przystosowania (ryc. 1 i 2). Omówiono z Klientem te trudnoci, których dowiadcza podczas dnia codziennego. Okazalo si, e najwicej problemu sprawia Panu Tadeuszowi samodzielna kpiel. The ongoing evaluation of independence while bathing confirmed big difficulties in performing this task (the result of the initial assessment made by the client: performance ­ 4; satisfaction ­ 4). The agreed adaptation of the bathroom to the client's needs included making a wooden step covered with an anti-slip mat (an easier way to go into the bath) (fig.3) and handrails fixed to both walls (making it easier to keep balance and protecting from a potential fall). The occupational therapist's task performed in cooperation with the client was to make him feel safe in his bathroom and be self-reliant while having a bath. Podczas tej sesji terapeutycznej ustalono zakres niezbdnych do wykonania zmian (adaptacji) w lazience, które odpowiadalyby oczekiwaniom i potrzebom Klienta oraz sposób w jaki mczyzna bdzie si uczyl z nich korzysta. Ocena bieca ­ samodzielnoci i niezalenoci podczas kpieli ­ potwierdza, e Klient dowiadcza duych trudnoci z wykonaniem tej czynnoci (wynik oceny wstpnej Klienta: wykonanie ­ 4; satysfakcja ­ 4). This method makes it possible to describe the goals of occupational therapy clearly in the following categories: S (significant) ­ describing the patient, M (measurable) ­ defining the tasks in the therapy, A (achievable) ­ defining the period during which the results should be achieved, R (realistic) ­ indicating activities planned within the occupational therapy with regard to the previously set goal, T (timed) ­ determining the time of working with the client until the reassessment of the results. T ­ two weeks During the second therapeutic session with the client on January 4th, 2011, the following steps were taken: ­ discussing the concept of adapting the bathroom to the client's needs which were previously reported; ­ providing the client with details on the adaptation, including the information where and how the equipment will be fixed; ­ taking measurements indispensable for manufacturing the step and rails (length, height, width); ­ establishing together with the client where exactly the rails should be fixed; ­ obtaining the client's and his wife's consent to realise the presented plan and implementing it. 5. Implement the plan The third visit at the client's house took place on January 29th, 2011. During that session the bathroom was adapted according to the previous plan. Handrails and a step were mounted (fig.4). Moreover, the manner of using the equipment was explained and practised (fig.5). Ryc. 4. Bathroom adaptation (handrails and wooden step covered with an anti-slip mat) Podczas drugiej sesji z Klientem, w dniu 4 stycznia 2011 r. podjto nastpujce dzialania: ­ przedyskutowano koncepcj wprowadzenia w lazience niezbdnych udogodnie w zwizku ze zgloszonymi uprzednio potrzebami; ­ poinformowano o tym, jak bdzie wyglda zaproponowana adaptacja, zademonstrowano w jaki sposób i gdzie zostanie zamontowana; ­ wykonano pomiary niezbdne do wykonania stopnia i porczy (dlugo, wysoko, szeroko); ­ ustalono wspólnie z Klientem dokladne miejsca zamontowania porczy; ­ uzyskano zgod Klienta i jego ony na realizacj przedstawionego planu i wykonanie adaptacji w lazience. 5. Wdroenie/wykonanie planu Trzecia wizyta domowa u Klienta miala miejsce w dniu 29 stycznia 2011 r. Tego dnia zostaly wprowadzone w lazience ustalone zmiany. Task: entering the bathroom, leaving crutches on the left side, grabbing the rail, standing and maintaining balance, getting into the bathtub, undressing, getting dressed, using a towel and bath accessories, learning, thinking, solving problems, using the right hand, following instructions such as `go towards the step', `grab the rail with two hands', `put your right foot on the step', `put your Ryc. 5. Poniej zanalizowano poszczególne etapy wykonania nauczanej czynnoci, wedlug obowizujcego w terapii zajciowej Strukturalnego Schematu Wykonywania Czynnoci (Taxonomic Code of Occupational Performance) [13]. 1. Aktywno ruchowa i umyslowa: wiczenie mini nóg, ramion i tulowia ­ zginanie, prostowanie, przywodzenie, odwodzenie, rotacja; aktywno umyslowa ­ koncentracja na zadaniu, zapamitywanie kolejnych etapów wykonywanego zadania. 2. Dzialanie: chwytanie i trzymanie si porczy, chodzenie z i bez uycia kul, wchodzenie na stopie, wchodzenie do wanny, utrzymanie pozycji siedzcej w wannie, ruchy obrotowe cialem; ustawiczne mylenie na temat wykonywania kolejnych etapów dzialania. 3. Czynnoci: chód, stanie, chwytanie, ukladanie, wchodzenie do wanny, ubieranie si, rozbieranie, uywanie rcznika i akcesoriów kpielowych, uczenie si nowych czynnoci i zachowa, mylenie analityczne, rozwizywanie problemów. 6. Monitorowanie i modyfikacja planu Ustalono z Panem Tadeuszem sposób kontaktu, decydujc si na kontakt telefoniczny i, o ile to bdzie konieczne, wizyty domowe. W tym czasie kiedy Klient testowal wprowadzone udogodnienie, systematycznie monitorowano i konsultowano jego postpy i ewentualne trudnoci z nim samym i jego on. Activity: remembering the performed activities, thinking according to a proper order, bathing without the help of others. 5. Occupation: walking, standing, grabbing, putting, entering the bathtub, getting dressed, undressing, using the towel and bath accessories, learning new activities and behaviours, thinking analytically, solving problems. 6. Monitor and modify It was agreed that the client would be contacted by phone and, if necessary, would be visited at home. At the stage when the client tested the implemented solutions, his progress and potential difficulties were monitored and discussed both with him and with his wife. It was checked whether the new devices were helpful in everyday bathing. The client's wife stated that during the process of learning how to bathe independently, he used the implemented equipment trying to remember successive stages of this activity. He rarely needed hints concerning what he should do and how at that particular moment. He learned how to perform the activity in the correct order, which helped him become more and more independent. He practised grabbing, moving without crutches and going up and down the step located next to the bathtub. At that time the client also practised regularly with his physiotherapist (strengthening muscles) and performed the recommended physical exercises by himself. After a few cases of getting into the bathtub with the use of the step, he decided that he did not need the anti-slip mat since he felt confident without it while standing on the step. Upon the request of the client, the mat was removed and it was the only modification to the previously established therapy plan. 7. Evaluate outcome The fourth meeting with the client took place on February 14th, 2011. Reassessment (using COPM) was performed two weeks after the implementation of the supporting devices. The client and his wife confirmed that the previously defined tasks (bathing independently) were realized and the goals were being gradually achieved. The assessment of performance and satisfaction increased by 3 points (tab. 2). The client said: I am very happy with the equipment that has been installed in my bathroom. Po wykonaniu kilku prób wejcia do wanny z uyciem stopnia, mczyzna uznal, e na t chwil nie potrzebuje maty antypolizgowej, poniewa czuje si pewnie na stopniu nawet bez pokrycia mat. Na wniosek Klienta zrezygnowano z uycia maty, i byla to jedyna modyfikacja jak wprowadzono do przyjtego planu terapii. 7. Ocena wyników terapii Czwarta sesja z Panem Tadeuszem odbyla si w dniu 14 lutego 2011 r. Powtórn ocen z wykorzystaniem kwestionariusza COPM przeprowadzono po uplywie dwóch tygodni od wprowadzenia ustalonej adaptacji w lazience Klienta. Uzyskano zapewnienie od mczyzny i jego ony, e okrelone uprzednio zadania (samodzielna kpiel bez pomocy) i cele s stopniowo osigane. Ocena wykonania zadania i satysfakcji na tym etapie terapii wzrosla o 3 punkty (tab.2). Klient powiedzial: ,,Jestem bardzo zadowolony z urzdze, które zostaly zamontowane w mojej lazience. W tej chwili nie obawiam si o to, e mog upa. Wiem, e kiedy bd tracil równowag, to mog chwyci si porczy. Czuj si z tym lepiej i jestem bardziej pewny siebie. Jestem bardziej spokojny i usatysfakcjonowany z tego powodu, e mog wykona codzienn toalet, poniewa jestem bardziej samodzielny podczas kpieli, golenia si i mycia. Stopie jest dobrze dopasowany i ulatwia mi wejcie i wyjcie z wanny. Porcz pod oknem umoliwia korzystanie z toalety (latwiej mi wstawa z sedesu). To byl dobry projekt, a wprowadzone w mojej lazience zmiany uwaam za funkcjonalne. S to "meble" pierwszej potrzeby, które pozwalaj mi bardziej ufa sobie samemu. Niczego lepszego w tej chwili nie potrzebuj". ona Pana Tadeusza uwaa, e ,,m jest teraz bardziej pewny siebie. W te dni, kiedy lepiej si czuje w ogóle moe kpa si samodzielnie. Wczeniej zawsze musialam mu pomaga. Jestem wprawdzie obecna w lazience, ale wylcznie go obserwuj i czasami tylko podpowiadam, co powinien zrobi. Kiedy m gorzej si czuje, pomagam mu postawi lew nog na stopniu. It was a good idea and I think the changes implemented in my bathroom are functional. This is necessary "furniture" which allows me to be more self-assured. I don't need anything better. His wife stated: "My husband is more confident now. It is important for him that when he is moving in the bathroom, he may grab the rail whenever he needs it. My husband is more independent while bathing. Now, I help him less frequently. When my husband has a good day and feels better, he is able to bathe himself. Previously, I always had to help him. Now I'm present in the bathroom, but I'm just watching him, and sometimes I tell him what to do. When my husband feels worse, I help him put his left foot on the step. I'm happy with the equipment which is aesthetic. Also, the rails have one more function ­ they serve as towel rails. I am glad that my husband is more independent while bathing. Conclusion The client is aware of the fact that his disease is progressing. He understands that with time he will become less physically fit. However, he confirmed that he was open to further cooperation with the occupational therapist and physiotherapist in order to maintain the level of fitness and independence. In the two-week period, the patient explored new possibilities and engaged in learning how to use newly fixed equipment. It was noted that the client was constantly looking for ways to improve his level of independence and perform tasks connected with using the bathroom better. Moreover, he regularly participated in the therapy with his physiotherapist and performed exercises at home. The man constantly looks for information concerning his disease and talks to other MS patients on the Internet (Facebook, blogs, etc.). He often discusses the disease with them in order to gain more information and help others understand it better and accept it. He is also a member of the Polish Multiple Sclerosis Society. The sessions with the client were also valuable due to the fact that issues other than those associated with the disease could be discussed with him, which was also significant for the client tycznie wykonane. Potwierdza jednak, e jest otwarty na dalsz wspólprac z terapeut zajciowym i fizjoterapeut, w celu podtrzymywania sprawnoci i samodzielnoci. Podczas tych dwóch tygodni przed Panem Tadeuszem otworzyly si nowe moliwoci i z duym zaangaowaniem uczyl si korzysta z zainstalowanych w lazience urzdze. Zaobserwowano, e Klient wci szukal sposobów na to, aby podnie na wyszy poziom swoj niezaleno i lepiej wykonywa zajcia zwizane z korzystaniem z lazienki. Ponadto regularnie uczestniczyl w terapii ze swoim fizjoterapeut, a take codziennie wykonywal zestaw wicze gimnastycznych w domu. Mczyzna stale poszukuje informacji o swojej chorobie, rozmawia z innymi osobami, które take cierpi z powodu SM, a czyni to za porednictwem Internetu (facebook, blogi itp.). Czsto dyskutuje z nimi na temat choroby, po to, aby wicej si na jej temat dowiedzie, ale te innym pomóc j lepiej pozna i zaakceptowa. Pan Tadeusz jest te czlonkiem Polskiego Towarzystwa Stwardnienia Rozsianego. W sesjach odbytych z Panem Tadeuszem wartociowe bylo te to, e mona bylo porozmawia z Klientem równie na inne tematy ni tylko te zwizane z chorob, co ­ jak zauwaono ­ bylo dla mczyzny bardzo wane. Podczas wspólpracy z Panem Tadeuszem zaobserwowano, e jest to pogodny mczyzna. Nawet kiedy odczuwa ból stara si umiecha i artowa. Ma wewntrzn sil do walki z chorob i wynikajcymi z niej ograniczeniami, chtnie te mówi o swojej chorobie. Kiedy powiedzial, e chce by niezaleny i samodzielny w zakresie codziennych zaj, tak dlugo, jak to tylko bdzie moliwe. A z tego powodu, e nie chcialby porusza si wylcznie na wózku inwalidzkim, to codziennie wykonuje swoje wiczenia, wzmacniajc minie nóg i rk. Jest wspierany przez swoich najbliszych ­ on i synów, którzy rozumiej trudnoci, których dowiadcza w codziennym yciu i pomagaj mu kiedy tylko tego potrzebuje. Zrealizowany z Panem Tadeuszem proces terapii zajciowej byl bardzo satysfakcjonujcy. It was also noted that the client is a cheerful person. Even though he felt pain, he was trying to smile and joke. He has internal strength to fight the disease and limitations resulting from it, and is willing to talk about the disease. He once said that he wanted to be independent and self-reliant in everyday life for the longest time possible. Every day, he performs leg and arm muscle strengthening exercises in order not to be dependent on a wheelchair only. His wife and sons support him as they understand difficulties which he experiences in everyday life and help him whenever he needs it. The process of occupational therapy realised with the client was really satisfactory. It was significant that the help provided to the client enabled him to perform everyday activities more efficiently and was adequate to his needs. Success and achievement of the goals of occupational therapy depends on a few factors, one of them being conscious and active engagement of the client. His openness and trust developed towards the occupational therapist were a starting point for good cooperation and exerted significant influence on the efficiency and effectiveness of the process. In a short period from the bathroom adaptation, the client was able to note and experience an improvement in his own performance and independence.
